Responsible for metric:  Associate Superintendent for Operations and Quality

Distribution:  Cabinet, Supervisors, Board of Education, Principals

When:  One week after report received from Worker Compensation carrier

Determination of loss time injury:  Reportable injury to worker compensation which results in absence from duty of an employee.  Each injury resulting in a loss of work time will be counted.  Each injury will only be counted once, even if loss time continues into the next month.

Trending:  Monthly points for two years

Data fields necessary:  Form 45 completion and submission to Worker Compensation carrier.  Data for graph from the report of loss time injuries from monthly worker compensation reports.
