Fort Dearborn Life-Dental Plan – pdf

The Dental Insurance Program is allowing an open enrollment period. Employees may elect to make changes in their coverage at this time.  The open enrollment period begins August1, 2010 and ends August 31, 2010. Insurance changes become effective September 1, 2010.

Open enrollment allows employees eligible for dental insurance benefits to add dependents or elect to accept coverage. 

Any individual desiring to make changes should contact Debbie Brooks or Julie Bennett, Administration Center between 7:30 a.m. and 4:30 p.m. An enrollment form will need to be completed.  Pre-exist conditions as declared in the plan document apply to any changes made at this time.
